The end-of-fieldwork BACB form submitted with your certification application, summarizing your total hours.
What it means
The Final Fieldwork Verification Form (F-FVF) is completed at the end of a fieldwork experience and submitted as part of your BCBA certification application. On it, the supervisor attests that the trainee completed the fieldwork in compliance with all requirements. Like the monthly form, it must be retained for at least 7 years, and applicants after Jan 1, 2027 use the 2027 version.

After finishing your hours, you submit your signed Final form or forms with your application.
Where it trips people up
The BACB refers to Final forms in the plural, so check with your supervisors how many your fieldwork needs. If a supervisor won’t sign despite the contract terms being met, the BACB has a Contested Fieldwork process.
